Usually yes, and usually at a flashing or transition rather than in the middle of the field. Wind-driven rain reaches details that ordinary rain does not. Say that it is storm-only in the request; it narrows the search considerably.
The tile looks fine. Can the roof still be failing?
Yes. On a tile roof the tile is the wear surface and the underlayment is the waterproofing, and the underlayment goes first. A roof that looks intact from the ground can be well past the end of its underlayment life.
